photos on a computer printer if you do not want to send an original photo.

I hereby grant to Masquerade Entertainment and Goddess of Discipline,
a.k.a. Empress Jina or anyone authorized by the organization(s) the right to  
reproduce, publish and otherwise use my photograph and
my working name on the internet for purposes of advertising.

in writing that such photos be removed and that they will
be removed within 30 days.

I hereby waive any causes of action I may have on account of 
the use of my working name or photograph(s).
If the photographer is unavailable for a signature, I warrant
that the agreement between the photographer and myself allows
for me to use the photos for advertising purposes.  Should 
the photographer bring a cause of action, then I agree to indemnify
for all costs of defending such an action.

These must be signed by any person(s) readily identifiable in the photo
and preferably the photographer . A parent 
or guardian must sign for those under age 18.
